We hit the streets to ask: Are you covered for public or private hospital?

In Episode 2 of Real Talk, Real Coverage, Tay Ping Hui heads back into the heart of Singapore to ask one simple yet crucial question: “Do you know if you are covered for public or private hospital?”

Public vs private hospital in Singapore — What’s covered by health insurance?

One of the main takeaways of Episode 2 is that many Singaporeans don’t realise their health insurance plans differ significantly depending on whether they go to a public hospital (e.g. Singapore General Hospital, Changi General Hospital, National University Hospital) or a private one (e.g. Raffles Hospital, Mount Alvernia Hospital, Gleneagles Hospital).

As Singaporeans or residents of Singapore, many of us will be familiar with the clear differences between public and private hospitals.

  • Public hospitals often come at lower bills and fall under standard MediShield Life frameworks, but may involve longer wait times or semi-private wards.
  • Private hospitals, on the other hand, offer faster and more comfortable care. Coverage falls under a private Integrated Shield Plan.

During our street interviews, we discovered that despite having bought an Integrated Shield Plan in the past, most people we interviewed could no longer remember if their policy covers private or public hospital stays or whether they would have to foot a small part of the bill or cover a significant portion.

The rising cost of health insurance premiums in Singapore

The second hot topic we chose to tackle are the escalating insurance premiums and how they’re impacting Singaporeans and PRs of all ages. Rising health insurance premiums come as no surprise considering the general inflation that we are all experiencing, rising healthcare costs, and Singapore’s ageing population.

  • Ageing risk pool means insurers must raise premiums over time.
  • Even younger policyholders are starting to feel the pinch, with regular renewal increases cutting into already tight budgets.
  • Yet, during our interview, few respondents seemed aware of rider clauses, caps, or annual premium increments.

This gap in understanding your coverage can result in unwanted shocks, uncertainty in the midst of recovery, and a mismatch between your healthcare choices and your budget.

How knowing your coverage protects you

We get it, insurance, particularly health insurance, isn’t the most thrilling topic. As adults, we’d much rather enjoy the perks of independence without stressing over fine print and policy jargon. But here’s the thing: you owe it to yourself to know what you’re actually covered for. Waiting until you’re hospitalised or facing a big bill is the worst time to find out the gaps in your plan. Understanding your coverage now means you can:

  1. Be prepared, not surprised. Know the details of your plan such as “What hospital type does my plan cover?”, “What are my rider limits?”, or “How do my co-payment rules work?”. This way you can work out what you’ll really have to pay upfront, if any.
  2. Make confident hospital choices. Ask yourself, “Does my plan fully cover private hospital fees?” and “Will I save more by choosing a panel specialist or public ward?” Understanding the answers helps you make informed decisions that fit your comfort level and budget.
  3. Plan ahead for premium changes. Consider, “How much might my premiums increase as I age?” or “Should I add riders or switch plans before costs go up?” Knowing these factors early helps you adapt your coverage to your future needs.

Make your coverage work for you

StepWhat to do
Step 1: Check your policy documentsFigure out what is included in your policy, namely, hospital type, ward class, rider limits, co-payment, and pre-/post-hospitalisation.
Step 2: Use toolsUse our Coverage Checker or the tools available on various websites such as CPF or even hospital websites that can help break down what you’re covered for (and what you’re not), and even what your estimated hospital bill might be.
Step 3: Talk to your agent or financial advisorAsk specific questions such as “Do I have coverage for private hospitals?”, “Does my plan cover pre- or post-hospitalisation visits?”, “Are my pre-existing conditions covered?”
Step 4: Plan aheadRemember to factor in premium increases while budgeting for your lifestyle and aim to review your policy every year to stay ahead.

The big takeaway: Know your coverage

Episode 2 has uncovered a common truth among Singaporeans and PRs, health insurance is often more complex than it seems. Many believe they’re fully prepared and well-covered, yet some find they’re only partially protected, or have private hospital coverage but still opt for public hospitals because they’re uncertain about their actual out-of-pocket costs.
